Renal Protection in Patients with DM and Advanced Nephropathy

Sheldon Tobe, MD, FRCPC, MScCH (HPTE), FACP, FASH
Dr. Tobe is a Nephrologist and Hypertension Specialist at Sunnybrook Health Sciences Centre in Toronto. He received his Master of Science, Community Health, Health Practitioner Teacher Education degree from the Dalla Lana School of Public Health in 2012. Dr. Tobe’s research in the 1990s focused on dialysis research, but by the end of the decade his focus shifted to blood pressure management to help keep patients off dialysis.
As former chair of the Canadian Hypertension Education Program (CHEP) his knowledge translation efforts have contributed to the dramatic improvement in hypertension awareness treatment and control in Canada over the past 20 years and the associated improvement in mortality from heart failure, stroke and heart attack. Based on his expertise in guideline development, Dr. Tobe was invited to co-chair the CIHR initiated C-CHANGE process to harmonize a core set of recommendations for vascular-risk reduction from eight groups of clinical-practice guidelines (Hypertension Canada, Diabetes Canada, Cardiac Rehab, Exercise, Canadian Cardiovascular Society, Obesity Canada, Smoking Cessation and Stroke). The C-CHANGE harmonized guidelines was endorsed by the Canadian Council of the Federation’s Healthcare Innovations Working Group in 2012 for implementation in primary care across Canada. A recognized leader in his field, his effective collaborations with researchers, clinicians, policymakers and Aboriginal people have had significant impact in Canada.
In 2018, Dr. Tobe founded CHEP+ which is physician-based, non-profit organization aimed at disseminating accredited CPD programs based on the C-CHANGE guidelines. CHEP+ has over 15 accredited programs on various topics related to CVD prevention and management.
