Cambridge Cardiac Care Centre is a full-service non-invasive cardiology Centre focused on prevention of cardiovascular illnesses, early & accurate diagnosis of cardiovascular risk & disease, as well as thorough & complete cardiovascular therapy & rehabilitation. Our expert staff will provide you on site counseling & education to help you heal yourself. Our state-of-the-art cardiac testing facility will provide you with rapid & complete non-invasive on-site cardiac investigations. Through our focus on lifestyle change & cardiac rehabilitation, we will work with you to develop a comprehensive plan to help protect you from cardiovascular illness or recover from it should it pre-exist. Position Summary:
The Nurse Practitioner works collaboratively with the Complex Continuing Care and with the Cardiac Rehab inter-professional team in the medical management of ASCVD out-patients. The Nurse Practitioner will be a clinical resource and will provide quality care through advanced practice, education, research, leadership and consultation as outlined by the Facility policies and procedures and the Guidelines for Practice from the College of Nurses of Ontario.
MUST HAVE PREVIOUS EXPERIENCE TO WORK WITH PATIENTS HAVING CARDIAC AND CARDIOVASCULAR CONDITIONS
Position Duties & Responsibilities:
Works within a defined scope of practice as an advanced practitioner within the auspices of approved medical directives and a practice agreement with physician partners
Devotes the majority of his/her time to direct patient care/issues, which includes monitoring of therapy modalities and interventions.
Communicates and collaborates with the team, and the inter-professional care team with respect to assessment, diagnoses, treatment options, consultation possibilities, and discharge plans.
Identifies and interprets diagnostic and screening tests in the assessment of common cardiac & other related health problems.
Initiates and performs a wide variety of investigations, diagnostic procedures and treatments from an approved list.
Prescribes and initiates medications that are defined within the Nurse Practitioner’s scope of practice.
Utilizes critical thinking and diagnostic reasoning skills in managing patient’s/client’s needs for counseling, pharmacologic and or complementary therapies based on their presenting symptoms in both chronic and episodic illnesses.
Acts as a clinical resource for staff if the cardiologist is not available.
Reviews all cases with attending cardiologist, either in person or virtually and develops a care plan in collaboration with the cardiologist.
Implements algorithmic care plans as developed in collaboration with the cardiologist.
Keeps abreast of current legislation, regulations, and professional issues withing College of Nurses of Ontario scope of practice.
Maintains continuing education in the areas of cardiology, diabetes, chronic disease, and area that pertain to the job description.
Maintaining CNO practice standards for NPs
Other areas of responsibility:
The NP will have other responsibilities including:
1) Participation and supervision of Clinical trials
2) Participation in other NP-led Clinics & Initiatives (e.g. Chest Pain Clinic)
3) Management of Acute/Chronic patient if the cardiologist is not available
4) Teaching role: Supervision of Medical students, Nursing & NP students, Physician Assistant trainees & CVT students, etc.
Skills Required:
Ability to think problem solve, prioritize, demonstrate proficiency in computer communications, self-directed, independent work. Demonstrated excellence in communication, organization, & collaboration. The nurse will work in cardiac rehab, work with other RNs in the heart failure clinic, and some clinical research responsibilities.
• Following and caring for patients post cardiac admission/event e.g. MI, PCI, CABG, Valve surgeries, TAVIs, etc.
• Following medically directed algorithms (e.g. Bloodwork, medication titration)
• Performing ECGs & Collecting vitals
• Answering patient questions by phone or in person or by Virtual care
• Responding to other physicians’ offices and Pharmacy Queries
• IV starts/ Drawing Blood
• Independent Limited Physical exams
• Cardiac Rehab: Supervise cardiac rehab classes
• Manage adverse and serious adverse events
• Group teaching of cardiac related topics
Nursing Intakes, and out-take visits for Cardiac rehab
Other duties may be added as clinic needs require.
Required Qualifications and Experience:
Master’s degree in Nursing, or a combination of education and experience and experience as sanctioned by the College of Nurses of Ontario, is required.
Current and valid registration in the Extended Class with The College of Nurses of Ontario and current registration in good standing with the College of Nurses of Ontario as a Registered Nurse.
Completion of an approved and accredited Nurse Practitioner Program.
Current ACLS and CPR-HCP, WHIMIS certification
Mandatory Disclosure: Yearly CNO NP license, and LAP must be provided to the employer.

Job Type: Full-time
Salary: $55.00-$65.00 per hour, competitive to ONA pay-grid
