Rate: $ 56.72 – $72.72 hourly
Unit Summary
St. Peter’s Hospital has specialized in enhancing the lives of seniors and those with chronic illness since 1890. A HHS complex care hospital on Maplewood Avenue in downtown Hamilton, St. Peter’s Hospital provides inpatient, outpatient, community-based programs and services that focus on Dementia, Aging, Palliative Care and Rehabilitation.
St. Peter’s Hospital is committed to finding ways to minimize and manage the daily effects of lifelong illness by getting to know the person behind the illness. A high-touch, low-tech approach to caring ensures that the physical, mental, emotional and spiritual needs of each patient are met every day.
The 63 bed Medically Complex Program provides care to patients with a broad range of complex medical conditions. These individuals require ongoing medical care, skilled nursing care, assessment and active management by specialized interprofessional teams. These patients often have extended length of stays related to complexity and chronicity of conditions. Advanced discharge plans with patients with extended length of stay are facilitated in collaboration with the community.
Position Summary
The Advanced Practice Nurse (APN) in Medically Complex ,is a registered nurse in the extended class (Nurse Practitioner –Adult) from the College of Nurses of Ontario (CNO)with the advanced education and specialized skills/ knowledge to provide clinical care grounded in evidence based standards for the hospitalized patients in the Medically Complex Unit, St Peters Hospital. The Nurse Practitioner works autonomously, and in a collaborative practice model with the physicians from the Medically Complex unit and/or physicians within the St Peters site, and in consultation/ collaboration with the interdisciplinary team, and with consultants according to their professional, ethical and legal standards outlined by the CNO. The components of the APN role include: Clinical Practice, Education, Research, Consultation, Leadership and Professional Development.

Qualifications
• Current Certification of Registration with the College of Nurses of Ontario as a Registered Nurse, Extended Class with Nurse Practitioner (Adult)
• Master’s Degree in Nursing and/or equivalent graduate Degree
• Minimum of five (5) years recent clinical experience supporting a population of acute medical /surgical patients; gerontology background preferred
• Demonstrated experience in research and program development
• Demonstrated excellent interpersonal and communication skills, and competency to work within interprofessional teams
• Demonstrated leadership and critical thinking skills
• Demonstrated ability to promote professional development in nursing
• Eligible for Faculty appointment with McMaster University
Accountable to:
Program Director of Complex Care and Orthopedics program and Interprofessional Practice Chief
Works conjointly with the Chief of Complex Care, Aging and Palliative Care and the Complex Care Interprofessional team in the development, implementation and yearly evaluation of a collaborative practice agreement and medical directives.
