Job Description

Position Title: Nurse Practitioner – Full Time Temporary (1.0 FTE) with significant opportunity for permanent placement.
Reports to: NDFHT Executive Director
Wage: Hourly $62.65
Benefits: The NDFHT is a participating employer of the Health Care of Ontario Pension Plan (“HOOPP”).

Position Overview:

The Nurse Practitioner (NP) will function as a member of the primary care team in the provision of primary health care services within North Durham Family Health Team. The NP provides primary care functions and participates in the development, implementation, monitoring and evaluation of programs and services of the North Durham Family Health Team. The NP will develop and utilize a broad range of community partnerships to develop and lead individual and group-based programs for patients.

Duties and Responsibilities:

• Provides evidence-based assessment, screening, prevention, and management to individuals utilizing best-practice guidelines.
• Autonomously diagnose, order and interpret diagnostic tests, prescribe pharmaceuticals and perform procedures within the RN (EC) legislated scope of practice, as appropriate.
• Perform health assessment and diagnosis; health care management and therapeutic intervention; health promotion and prevention of illness, injury and complications in the care of complex medical patients, pediatrics and geriatric patients.
• Facilitates application of relevant information to make effective judgments and decisions to lead the management of primary care.
• Referral to community programs and specialists when appropriate.
• Shares in the provision of program management & leadership, referral, and care coordination.

Job Requirements

Qualifications and Relevant Experience:

• Master in Nursing/Primary Care Nurse Practitioner Stream
• Current registration from the College of Nurses of Ontario as an RN (EC) – Nurse Practitioner-Primary Health Care (PHCNP) or Nurse Practitioner-Primary Care
• Member in good standing of the College of Nurses of Ontario
• Professional Liability Insurance
• A clear criminal record check with vulnerable sector screening
• BCLS required
• Membership in the Registered Nurses’ Association of Ontario & Nurse Practitioner Association of Ontario, preferred

• Evidence of program design, resource development and project management skills.
• Working knowledge of the College of Nurses of Ontario standards of practice
