Job Description

This is a full-time position with the Wawa Family Health Team.

The Nurse Practitioner (NP) will function as a member of the primary care team in the provision of primary health care services in the Wawa Family Health Team. The NP provides services to clients who include the individual throughout their life span, the family, and the community, with a strong emphasis on health promotion, patient education, and disease prevention. WFHT Nurse Practitioners do not roster but can be MRP for their admitted patient next door at the LDHC.

The NP provides primary care functions and participates in the development, implementation, monitoring and evaluation of programs and services of the Wawa Family Health Team which include child wellness checks and immunizations, women’s health, prenatal, antenatal, and postpartum care, transgender care, addictions, and mental health.

Job Requirements

Accountability
● The Nurse Practitioner is to practice within the scope and according to the standards of practice as outlined in the RN-EC scope of practice provided by the College (www.cno.org);
● The NP functions within the policies and procedures of the Wawa FHT;
● As a primary care provider, the NP is accountable to clients by maintaining confidentiality and acting in an ethical and professional manner, in accordance with the professional code of ethics and in accordance with the policies of the Wawa Family Health Team;
● The NP is accountable as a member of the WFHT, to obtain appropriate physician consultation according to the College of Nurses of Ontario Expectations for Consultation with Physicians by NP.

Skills/Abilities
● 2-3 years of clinical experience in the NP scope will be considered an asset;
● Strong health assessment skills;
● Strong organizational skills;
● Knowledge and proficiency in current, evidence-based methods and practices of primary care delivery, with an emphasis on attaining the organization’s strategic goals;
● Good time management, communication, and interpersonal skills;
● Demonstrates autonomy while assuming and having sole accountability and the authority to independently communicate diagnosis, prescribe drugs, and order a range of diagnostic tests within the scope of an NP;
● Desire and ability to update knowledge and skills through various means including technology-based opportunities, courses, workshops, and conferences;
● Exhibit the ability to be open and non-judgmental;
● The NP maintains knowledge of the goals and structure of the organization, the policies and procedures and the programs.
● Superior communication, problem solving and conflict resolution skills

As the WFHT is established and always welcoming innovative projects and strategies to meet the primary care of our community. The Nurse Practitioner needs to be flexible and able to adapt as the organization progresses.

The NP role is responsible to work within the scope of practice outlined by the College of Nurses of Ontario and within the policies of the Wawa Family Health Team at all times.
