Health Care Systems to address Dementia and Complexity
Nurse Practitioners are Pivotal

- Increase awareness of system wide impacts of dementia. Tools for regional planning.
- Comprehensive Geriatric Assessment: tools to support complex care of older adults.
- Systems of care to support dementia evaluation, diagnosis, and care.
- Evaluation of current Central East systems of care.

Dr Jenny Ingram, MD
Biographies:
Stacey Moore: Nurse Practitioner BA, BScN, MN-ACNP, RN(EC)
Stacey Moore has been an Adult-NP since 2009. Her practice focus has been in hospital based outpatient care, including TIA-Stroke Secondary Prevention Clinics and Specialized Geriatrics. Stacey also has a background in intensive care and cardiac surgery at Sunnybrook Health Sciences and was the co-owner/operator of a multi-disciplinary health centre in Oshawa. Stacey is currently the NP lead with the Peterborough Regional Health Centre Geriatric Intervention and Assessment Network (GAIN) clinic, where she has been working for the last 10 years.
Dr. K. Jennifer Ingram MD, FRCPC
After graduating from St Clements (in 1968), Jenny studied Medicine at the University of Toronto and Queens University. She became a Specialist in Geriatric Medicine at the University of Western Ontario in London,
Jenny and Alan moved to Peterborough with their three children ages 1, 7 and 10. Soon, Jenny recognized seniors from Scarborough to Belleville and north to Algonquin Park were seeking her medical involvement. She thus founded a research and memory clinic in Peterborough – the Kawartha Centre. Dr. Ingram successfully sought local partners resulting in the funding of 12 interdisciplinary care teams across Central East Ontario, called GAIN Teams and the Peterborough Council on Aging, now a part of our municipal government. Currently Dr. Ingram is working with Trent University and the Peterborough Family Health Team to improve access to diagnosis of Dementia through improved technology and community agencies working with all family doctors. Dr. Ingram was awarded the 2007 Trent University Community Leaders Award, and the OMA Community Leadership Award among other accolades.
