POSITION OVERVIEW
Provides on-call coverage as the Most Responsible Provider/Prescriber (MRP) for residents of Andy’s House and works within her/his legislative scope of practice as described and outlined by the College of Nurses of Ontario (CNO).
Oversees Andy’s House at Home Program and provides Leadership-related activities, as well as clinical oversight, referrals and consultations for recipients of care that meets the same quality that exists within Andy’s House.
Establishes strong relationships with community organizations, and community partners in order to ensure appropriate levels of support, and standards of practice comply with provincial models for palliative care.
COMMUNITY PROGRAM DIRECTOR
· Collaborates with community and regional partners to develop, implement and evaluate a secondary level, community- based palliative care program in Muskoka that complies with provincial, HPCO and OPCN standards of practice for palliative care.
Performs program audits and analysis to demonstrate and report positive outcomes, satisfaction, safety and fiscal accountabilities.
Directs staff who report to her regarding performance expectations and achievement and coaches them to develop skills required for successful program delivery.
Participates as part of the Hospice Muskoka Leadership Team to ensure organizational excellence and sustainability is achieved.
Ensures professional development and clinical competencies are maintained through on-going education and training.
DIRECT CARE TO RESIDENTS
· Provides comprehensive palliative care to residents as part of the interdisciplinary care team by:
a. Collaborating with the resident and family/caregiver, and the health care team in the development, implementation, and evaluation of the resident’s plan of care to achieve goals of care as defined by the palliative person
b. provides leadership and mentorship to staff to enhance their knowledge, assessment skills, and ability to care for residents at end-of-life;
c. leads and collaborates in research, education, and evidence-based practice initiatives to optimize the resident’s experience.
Performs a person-centered health assessment of residents on admission, and as needed (including weekly medication reviews). Conducts person-centered comprehensive health histories.
Engages with residents/SDM (substitute decision maker) in regular dialogue about their goals of care. Participates in care conferences.
Orders and/or performs appropriate screening and diagnostic investigations, interpreting results and assuming responsibility for follow-up.
Manages the care of residents by providing pharmacological, complementary and/or counseling interventions, and performs procedures within the NP scope of practice.
Participates in providing after hours and on-call coverage in accordance with the on-call policy of the Hospice.
TEACHING AND COACHING
Provides formal and informal teaching and coaching by integrating best practices in the management of clinical care to interdisciplinary team members, serving as resource person, educator and role model.
Conducts and contributes to the performance reviews of other staff who are direct reports.
Provides clinical placements and precepts NP students interested in specializing in palliative care or who are enrolled in a Post-Masters Certificate in Palliative Care.
Participates in identifying, analyzing, and interpreting trends in resident care outcomes and professional nursing practice issues to determine priorities for educational programming.
Contributes to planning, implementing, and evaluating learning resources and health education programs for residents, families, and substitute decision makers.
LEADERSHIP
Resolve various problems and related complaints and implement corrective measures and process improvements; working with Human Resources as needed for further problem solving, guidance, and conflict resolution.
Submit monthly program reports to the Executive Director, work with the Medical Director to ensure policies and procedures specific to Andy’s House at Home are developed and aligned with the HPCO Accreditation process to ensure full compliance.
Participate in Muskoka and Area OHT committees and work with health system leaders to contribute to the development of accessible, equitable and expert service delivery for the population of Muskoka.
Present at conferences, contribute to journals, and present to Committees of the Board from time to time.
Demonstrates exceptional data management (i.e. Meditech expanse) and analysis skills.
Demonstrates fiscal responsibility and people management skills throughout the implementation of Andy’s House at Home.
RESEARCH
Engages in evidence-informed practice by critically appraising and applying relevant research, and theory in providing health-care services.
Collaborates with members of the interdisciplinary team and/or community to identify research opportunities and to conduct and/or support research.
Acts as a change agent through knowledge translation and dissemination of new knowledge that may include formal presentations, publications, informal discussions, the development of best practices, policies and procedures.

Job Requirements

QUALIFICATIONS
Education (Degree/Diploma/Certifications)
Degree/Diploma in Health Care Administration or related field (Considered an asset).
Certified in Hospice Palliative Care Nursing (C)anada (CHPCN(c)) Considered an asset)
Current registration in good standing with the College of Nurses of Ontario (CNO) as a Registered Nurse in the Extended Class and licensed to prescribe Controlled Substances and Narcotics.
Palliative Care Facilitated Access (PCFA) certification.
LEAP certified; facilitator candidate (considered an asset);
Experience
In-depth understanding and minimum 5-years-experience in Hospice Palliative Care.
Experience in not-for- profit sector (Considered an asset).
Experience in rural, community-based care delivery models. (Considered an asset).
Knowledge, Skills and Abilities
Consultative team-based management style and strong problem-solving skills.
Effective verbal and written communication skills.
Ability to work independently, to exercise sound judgment, to make appropriate decisions and to accept responsibility for outcomes.
Positive approach to client service and a demonstrated capacity for empathy.
Commitment to teamwork and ability to collaborate effectively.
General
The ability to work flexible hours to accommodate meetings and to be accessible to staff and clients.
Be a strong member of the Leadership Team who demonstrates and creates respect and trust within the team; demonstrates support, active listening, and mentorship to clinical staff members.
Strong Computer skills in Word, Excel, Access, and PowerPoint.
Valid Driver’s License, use of reliable vehicle and willingness to travel.
Provide current Vulnerable Sector Police Record Check.
Is fully vaccinated against/immune to Tuberculosis (two-step Tuberculin testing only), Measles, Mumps, Rubella, Varicella (chickenpox), Hepatitis B, Tetanus/Diphtheria/Pertussis (whooping cough), Influenza, and COVID-19.
